Making Referrals

SpineOne makes patient referral easy. We accept referrals from physicians and surgeons, nurse case managers, claims personnel and attorneys. We ask that referring physicians and surgeons specify if you’d like to remain the primary treating physician or prefer to have one of our doctors assume patient care. We’ll obtain all authorization for consultations and treatment for you. Please follow these four easy steps:

1. Please fax insurance demographics, prescription, medical reports including PR-2s and narrative reports, if any, to 408.264.5894.
2. Please use a SpineOne prescription form (Download Here) to delineate the type of treatment desired and the healthcare provider making the referral.
3. Please send any reports or films of MRI’s, x-rays etc and operative reports.

SpineOne will keep you updated with authorization and the status of you patient. We accept self referrals. Please contact our office for more information at 408.264.1021.
